I signed up to Spam-hero in 2014 after trying countless local anti spam packages on our network and losing my mind with frustration. The impact was immediate and amazing - the level of control is brilliant with easy to use dashboards and helpful setup instructions it took me half an hour to configure with my LDAP server.
Nearly 3 years later i can honestly say it's the best decision we made as 99.92% of our spam never gets to my mail server. Specific things I like: Geo filtering, LDAP sync & bulk re-deliver - this has saved me twice when my mail server
crashed.

I'm not sure. I suspect that Google Workspace and Office 365 was catching the bulk of it and as an IT consultant (computer guy) all of my customers are on one or the other.
Filtering accuracy

I believe it is very accurate. More importantly, it gives me the big picture and lets me drill down on a particular piece of spam where I can potentially spot why it is being flagged that way. I don't get this big picture otherwise. And this big picture spans all my clients and all their email domains.
Ease of setup

Email is complicated. However, this spam filter setup process is not as it prompts you through each step of the way, and it can do a sanity check of your configuration before moving to the next step. Getting stuck, I have asked many questions on the live chat system where there is always someone available and responding within 60 seconds, I can't think of a time when I did not get a live response from a tech within 60 seconds and I keep some crazy hours.

Everything is just done right. I really like 'Sender Verification Details' where I can see the DMARC, SPF and DKIM status of an email. When trying to implement DMARC on a domain this insight helps significantly, frankly without this feature I may have failed once again at my DMARC compliance efforts.
